Please use this portal to report the date and time of your seizure (or seizure-like event). After you provide this information, someone from Science 37 will contact you to schedule your blood draw. Cognizance Biomarkers has partnered with Science 37 to collect blood for this research.

Please remember that the blood draw must take place within 72 hours of your seizure (or seizure-like event). The person performing the blood draw will meet you at your home or another place of your choosing.
